Located in the New Medical Office Building, STE 502

Bluegrass Community Hospital Rehabilitaion and Sports Medicine Center provides inpatient and outpatient care to communities of Woodford and Anderson Counties. We utilize techniques such as manual therapy, theraputic exercise to promote ROM and strength, modalities such as electrical stimulation, ultrasound, traction, moist heat and ice, and educatuion to prevent further injury to assist our patients in meeting their rehabilition goals. Some of the other services we provide are postoperative Physical Therapy, sports retraining, orthotic fabrication, post mastectomy rehabilitation, work conditioning/work hardening, job description/job site analysis, and wound care.

Important Information:

Hours of operation for clinic are:

• Monday – Friday, 8:00 a.m.- 6:00 p.m.
• Services are provided for both hospital inpatients & outpatients by licensed Physical Therapist.

A written Physicians order is required at time service. The order needs to include the following information:

• Patient’s name
• Referring MD name
• Diagnosis
• Patient’s phone number (if faxed order)
• Type of insurance coverage

Patients register in the Clinic itself: it is not necessary to go to registration first.
