As a healthcare provider, Bluegrass Community Hospital is subject to certain regulations in which we have to disclose your health info. These disclosures are generally routine to all patients and are done without your specific authorization for several reasons. State and Federal laws require us to report cases of abuse, neglect or other legal requirements. These laws require notification to:

Law Enforcement
Health Oversight Agencies
Death and Funeral Arrangements
Special Government Functions including public health or safety. Public Health Agencies
Judicial and Administrative Proceedings
Organ Donation Agencies
Military and Veterans functions to prevent serious threat to our community

We may also contact you after your current visit for future appointment reminders or to provide you with information regarding treatment alternatives other health related services with may be helpful to you. We will obtain your written authorization for disclosures beyond the reasons above. Do remember, if you do authorize us to release your information, you always have the right to revoke that later. We will be happy to honor that request except to the extent we have already acted.

Access To Your Health Information
In most cases, you have the right to look at or receive a copy of your health information. It may take up to thirty (30) days to respond to your request and there may be a preparation fee.

Accounting Disclosures
You have the right to ask for a list of instances in which we have disclosed your information for other than treatment, payment or operational purposes. We will provide one list per year without charge. All other requests within the same calendar year will subject to a nominal fee.
